Anyone trying out for a sport must turn in to the coach, on the first day, a completed Physicians Form and Concussion Form. (Please turn in directly to the coach; do not email or bring it by the school.) No exceptions! For those that are selected to be on a team, there will be a $50 sports participation fee.
