Seventh Grade Immunizations (required for school attendance)

A​ll students entering the 7th grade are required to have one dose of T-​dap (tetanus, diphtheria, pertussis) and one dose of meningococcal conjugate vaccine (for those who have not already received it – possibly before entering the 6th grade).

Please contact your child’s healthcare provider or your local health department to talk about which vaccines your child may need and to schedule an appointment to receive these vaccines. Please ask your healthcare provider or your local health department for a copy of your child’s immunization record. This copy will need to be given to the school on the first day. There will be a 30 calendar (not school) day grace period after the start of classes on ​August 16th to get the required​ vaccine(​s​)​ for your child.

If proof of the vaccine(​s​)​ is not provided before the​ start of school on Friday, September 14th your child will be unable to attend classes until proof of the vaccine(s) is provided to the school.

​If your family has religious reasons for choosing not to vaccinate, you must have both the Religious Exemption Form I and Religious Exemption Form II signed and ​on file by the September 14th deadline.​
